La malattia è anche conosciuta con i termini latini variola o variola vera (dal tardo latino variŏla, derivato da varius, a significare "vario", "chiazzato"), mentre il termine inglese smallpox venne coniato nel Regno Unito nel XV secolo per distinguerla dalla sifilide, denominata great pox.
